Financial Policy

Foundational Health is not currently contracted with any commercially available insurance companies.  We are a fee-for-service model and we request that all payments are rendered at the time of your appointment.  We will provide superbills as needed for you after your visit is complete, so that you may be reimbursed by your insurance company for your out-of-pocket costs.

Labs and most medications are typically covered by your insurance policy, dependent upon your insurance company and your deductible.

Cancellation Policy:

We understand that situations arise in which you must cancel your appointment. However, we request that you provide us with 24 hours notice for cancellations. This allows other patients to be scheduled into that appointment slot.

If an appointment is canceled with less than 24 hours notice, the deposit will serve as a cancellation fee to cover a portion of the lost time and resources.  The deposit will then be applied to the rescheduled appointment.
